Given the above, the Trials being so "regulated" doesn't make sense.
You can say that the Trials we play are merely a unique selected subset of all the goings on in the Entity's Realm.
You could also say that the Trials we play are guided by a "Mad Architect" intent on bending the Realm of the Entity for some unknown purpose.
You could also say it's both.
Of course now that I think about it I don't know what OP's post meant by "The Realm", if it was just the Trials or everything.

While it's possible for memories to manifest through thought in The Fog, the assumption that The Fog consist only of memories is incorrect, outside of Killers & Survivors who are trapped in The Fog along with the Fog Creatures created by the Entity itself, The Fog actually consist purely of Auric Cells only.
The Entity picks and chooses what does what when it looks at a memory of an area and thinks "this would work as a trial ground" but it does not understand why a house needs to look like a house, hence why we had stuff like old haddonfield houses. You can think of The Entity like an AI that generates content, it can create stuff out from what it interprets it as, but it doesn't truly understand what it's creating, which is why some stuff is so off.
So therefore, it makes perfect sense for the trials to be regulated. If The Entity wanted to outright KILL the Survivors, it wouldnt make it fair at all. but it needs everyone to be kept in an emotional state at all times in order to extract as much Sustenance as it can from both Killers and Survivors.
Which is why the trials are regulated. why pallets exist, the exit gates, etc.
Besides, The Fog and everything within it including the trials as well as all Fog Creatures and the insects and animals like Rats & Crows are all part of The Entity itself since they're all extensions of it, so why wouldn't it control every part of how the trials looks and functions when it's already in full control of it all? (Excluding Survivors and Killers.)

I can't really say the trials have anything to do with any Mad Architect because there doesn't exist one canonically, unless you're thinking of The Mad Designer aka Jezabel who gets mentioned every now and then in the tomes, however she has nothing to do with the trials themselves and are more about drawings, sketches and journals about scenes from other realities or about future events yet to happen.
Perhaps you've looked at the Mad Designer and mistaken them for something like a Mad Architect.

Survivors would no longer be dredged up from the Void, at least, not in an "intelligent" manner now. If there was a PvE portion to this game, the constant unchecked dredging of the Void would be the reason behind void monsters coming for characters.
The Entity's Realm is its own dimension and an infinite fractal across the omniverse. The Architects sub-domain is also a fractal, and their Tools now open to all who were once their experiment.
In my opinion, anyone willing to reverse engineer the nature of Auric Energy, and the Architects Tools, would be able to make claim to a new sub-domain. If they used the Architects Tools, they would also be connected to anyone else using the Tools from an Auric connection, having easy access to one another (for good or ill). Some might try exploring the Entities Realm, likely forever being lost.
This all sounds optimistic, but don't forget that in the Entities Realm your loves and more specifically your fears, will become manifest, in a literal sense. And boy howdy fear has a really nasty feedback loop.
Where in the lore is there talk about an Architect of the Trials other than the Entity?
As far as i'm aware from having read the lore, The Entity itself is the only thing that actually creates the trials themselves.

It's not the same. You will get lobby dodged for not showing your loadout.
Also - this feature is by no definition accessibility. It does not help certain group of people play the game. It does not alleviate any disability or anything like that. This is purely just about balancing soloQ with SWF to be closer together (a good thing still. It's just not accessibility feature)

If bhvr does a Walking Dead chapter and they don't go for the Telltale games, then i don't care about it.
I haven't played Last of Us, so that one is a literal whatever for me.
Chucky is probably the only candidate on the list that i care enough about to see added to the game, my utter craving for more pre-2000 horror movie licenses (both mainstream & niche) and lack of them in recent times have been driving me nuts, granted it's only been a year since we got Ringu but it somehow feels way longer than that.

Even this could be solved easily. Let survivor pick what other survivors see and let them have something else too (e.g. by having "lock selection" button - after which u click on loadout 2 where u have solo survivor, no mither, left behind and wakeup).
Overall there really needs to be done something to help soloQ. It's too rough right now
If Survivors will just see different perks from the ones you actually carry, then your solution is literally no different from just having the option to choose if you want to just outright hide what perks you carry by just not showing any perks at all because in both scenarios, the information is hidden away for your teammates.
Solo Q could definitely use some more features, however if we're gonna go with the feature suggested by OP, then it needs to be just that, a toggleable accessability feature for both perks and items seperately if you want others to seem them at all or not.

True wesker heads only plays a match if there is a Chris playing as well, they will then only play for 7 minutes and obsessively go after the Chris to mori them just for the special dialogue.

The fog itself and it's many realms are part of the entity itself and without the entity's presence and control over it all, those realms would collapse and perish.

A chain tethered spear gun doesn't exactly fit the typical horror mold, but Deathslinger has one.
We're talking about a piece of equipment that is canon to Nemesis so it matters little in terms of thematics.
The reason we don't have it is because it would be a nightmare balance and performance wise. Same reason we only get two zombies, same reason they didn't make The Crusher original character whose power was gonna be busting through walls. The game is quite limited in what it can handle.
Thing is, The Redeemer was designed specifically to fit into dbd's gameplay, you still need to m1 Survivors when they're reeled in, so it does still fit within the boundaries of dbd and it's themes since it's not like a regular projectile weapon.
Things like regular guns and explosive weapons does not fit dbd thematically, never has and never will, it does not matter how iconic it is to a character.
Also even Mathieu Cote said it himself back during the release of the first Resident Evil dlc in an interview how weapons like that doesn't fit dbd and pointed out how Deathslinger has a gun but it's specifically designed to fit into dbd.

Rocket Launcher will never happen because it doesn't fit dbd thematically, best you could do is give him some haste but that's about it.

devs have stated in the past that the chase music masking Survivor sounds is an intentional balance decision to give Survivors a chance to escape the killer.

Honestly, yeah they should give myers' mori that as it honestly would add to the immersion.
On the subject of music ques, they should re-implement this for myers when stalking someone but only make it audible for the one playing as Myers.

It depends on what killer i'm playing and if the map benefits them mostly.
My favorite maps are Midwich & RPD for their map design, with Garden of Joy as my favorite map in terms of aesthetics.

This should be more of an accessibility option than a forced implementation, because i'm fairly certain that Survivors will lobby dodge any teammates who's carrying perks that they don't want them to carry, especially No Mither.

That's not something they can change though, it's been well known that valve is pretty lenient with letting other companies use their content in crossovers in their own game, however the companies that uses valve's content for their game must release it for free and is never allowed to make money off of it and bhvr is no exception to this, it's been like that for a long time now.
The other thing that prevents it from happening is bhvr's reluctance to do free chapters anymore, however maybe if there is enough demand for it to happen, they might make it an exception for this, what is extremely unlikely however is bhvr adding any type of l4d related legendary skins.

I think there's only one game that doesn't use Insanity in some form, and that's A Machine For Pigs. Rebirth has the whole Fear mechanic, which is slightly different but essentially the same.
Yeah just checked, Rebirth does have the insanity feature when looking at enemy so in that regard, i stand corrected. With that said however, my statement still stands that you could still make it work in-game.

Well that depends on what amnesia game we're gonna adapt as a chapter because as far as i'm aware, only The Dark Descent has the insanity mechanic when looking at an enemy, which yes you can still make such a mechanic work in dbd.
Personally however, i'd much rather see Amnesia: Rebirth be the pick for an Amnesia Chapter.

We all love a good chase; I'm not talking about loops where I'm standing still waiting to go in the opposite direction of the killer. I'm talking about actually having both sides vaulting through obstacles, that red stain getting closer and closer building tension, with the lovely music. (Thanks, Michel F. April.) You may call this "holding W" well, no, I actually tend not to do that as I want the fun to be had for both of us, meaning vaults through buildings, using balanced and falling from ledges etc.
While I know Dbd is not Warframe or Death Garden, I do hope someday we can have a fun movement mechanic that is more involved than just the two keys. But hey, it keeps me going for what it is!

What kind of a mechanic would be considered a "fun movement mechanic" that would make chases more fun and not just be completely broken for one side or just be completely useless and not really add anything significant enough to the chase itself that would justify it's existence?

Everything in old dbd is a downgrade to what we have now and going back to it would mean that i'd be facing old Billy again, who is the killer i hate the most next to Blight.
Also not to mention playing against the snoozefest that is old freddy again.

Sex appeal has its place in horror as well. Always has. Humans and they monster they create are often used to express concepts around sex and sensuality. People can be attracted to the more seductive qualities that lay within horrifying individuals and some monsters are built around that aspect.
Really all I'm saying is thirst has always been in horror and dbd is fine theme wise. It has something for everyone and if you're not into what is out now, give it time you'll get something you like later. Doesn't have to be huge doom and gloom that dbd's fallen off the pure path of horror. This game could be the most sterile and grim experience possible, like a warhammer 40k game, and people would still find someone or something to thirst over. At least horror has space for that crowd to be serviced without harming anything else.
I think the biggest problem about the horror community in general are the people who loves gatekeeping what is and isn't considered horror for any kind of reason, it's kinda the same vibe i'm getting from some of the people in the dbd community whenever something even remotely attractive or not conventional horror looking is added.

I never really saw Vittorio as being thirst bait, personally i actually really enjoyed him for his lore and what he brings to the narrative of dbd's overall lore as well as his cosmetic potential, i'd quite frankly pick him over likes of other Survivors such as the Lyra twins any day.

One of the most overrated events imo, it always felt like a copy and paste of the Howling Grounds event which i enjoyed way more.

Realistically it's never gonna happen since bhvr wouldn't be able to sell any valve related content for money, but i'll humor the idea.
What we need are cosmetics for survivors that makes them look like Nick, Coach and Louis since those are the ones we're missing.
As far as map & killer goes, the final level of The Sacrifice campaign as the map along with a tank killer would be the most fitting since the Survivor we have is Bill after all and that's where he died and what killed him in the end.

As far as performance goes, while they aren't the strongest, they also aren't the weakest. There are other Killers that could use that attention more urgently, especially since over time fewer and fewer people will have access to that Chapter's content. With only so much bandwidth for changes in each update, priorities are a major factor.
Speaking of killers who could use attention more urgently, is the Twins Rework still happening? Been a while since we last heard anything about it.

A bit obscure, but I'd say a Entity-touched version of The Eye Of Adam from "The Cat Lady". A physically disabled person who spent time online convincing depressed people to kill themselves and succeeded, this "parasite" as he was called could have his abilities enhanced to maybe briefly control survivor movements, make them walk towards him or create mini nightmares for survivor.
However, he may have issues with mechanics such as vaulting (I believe he's confined to a wheelchair and almost fully paralysed) and carrying survivors, plus his story is perhaps too dark and may trigger things in others.
I'd also say The Distortion from "The Magnus Archives", which is an avatar of The Spiral deity. It's power could involve throwing people into pocket dimensions or warping the area to shift the structures of loops or items. However, this could be mechanically game-breaking and the Distortion itself is so crazy in appearance that it may even cause seizures.
Lorewise though, The Magnus Archive would fit really well into dbd's own lore since it's already been established that the black vale uses things like signals and horror podcasts among other things to transmit encrypted messages and information to one another about things such as the locations of bleeds and other supernatural stuff.

I rather see a QOL for his current kit & addons that doesn't involve bring aspects of old freddy back in any form or capacity.

I don't really care what they bring us as i know the IP i want to see the most isn't gonna happen anyway.
Best i can hope for is that it's another horror movie chapter.
Worst case scenario, it's another resident evil chapter or a second chapter to another existing license we already have.

Here's what i don't get about threads like these, why would you assume it's possible for Kayako to even be a legendary skin for another licensed character like Sadako when they're two completely seperate IPs? Not to mention, It would be the equivalent of giving Michael Myers a legendary skin of a scream related ghostface just because of a few similarities despite being completely different characters from one another with completely different powers & abilities overall.
Also to answer your question, of course Kayako should be her own killer, makes no sense not to do that considering Ju-On is pretty popular over in Japan just like Ringu is, i mean they even made the Sadako Vs. Kayako movie.

Base Haddie has had some run ins with ravage monsters though judging by her scars. An archive could definitely expand on that and bring the 2 versions closer together
I think the reason Tome Haddie isn't in the fog yet is because her story hasn't concluded yet given how they're still continuing her storyline in the npc tomes.

Tome Haddie has fought monsters ranging from Abominations with hundreds of eyes lurking in caves to Vampires that's been sealed away for a long time which can possess humans when released, She's killed people from the Black Vale & the monsters she encounters, she's gathering an army of people who can see into the bleed like her which she has named her group ''The Twelve to Midnight'', she's found remnants of characters such as Carmina and Vittorio, she's looking to stop the black vale from sacrificing the world to the entity and is overall a badass.
What does Base Haddie have to make her stand out from Tome Haddie and be just as cool as her? Pretty much nothing, that's why i consider her a poser to Tome Haddie.
Also i still wanna see a tome focusing on the Haley & Jaden versions of Haddie & Jordan in their airship traveling the fog, last time we saw Haley was when she met Saku in one of the Observer Towers before skidaddling back to the Airship and left in a hurry.

It's pretty easy to do that actually, it's tome just have to focus on the memories of the Ottomarians themselves, even the addons are about the objects that the people in the fold had, so it would make sense for the tome to focus on the people themselves.
Speaking of Ottomarians, i'm pretty sure that the Stamper from Dredge's story isn't the same Stamper from Doctor's story as they're from different terras.

Because it's Base Haddie, Tome Haddie is the superior Haddie and it pisses me off to no end how Base Haddie was picked over Tome Haddie for the chapter.

According to Mandy, those features were part of a larger update and the iterations of last man standing, self pickup and end game mori were early versions that the designers needed feedback on for further development.
It will most likely be back in a future update.

Most things i'd rather stay buried and forgotten, only things i kinda want back are the first reworked version of Lery and Oni's ptb chase music.
Out of all the things i despise the most and don't ever want to see re-added to the game, these are my top 10.
- Old Freddy (I feel nothing but scorn and disdain for your existence.)
- Old Billy (Genuinely pleased that he was removed.)
- Original Hatch Mechanic (Nothing but a time waster when it came to hatch standoffs.)
- Hawkins (Worst indoor map.)
- Old animations (Looked utterly clunky and bad.)
- Old graphics and aesthetics (Hated everything about it, especially the map tints.)
- All reworked Killer's original kits or powers
- All of the older versions of perks
- Original mori function
- Moonlight Offerings

Overall low expectations in general.
It's the best way to avoid disappointments while hoping for the best.
