Steam usernames, just like nametags on console, are handled by their own platform, so if you think one is insulting, report them to Steam.
Regarding the rest, targeting is griefing, if someone is specifically targeting some players and they get reported multiple times with evidence, that can lead to a ban.

As others already told you, to report a player always keep in mind in-game reports are mandatory (and as you said you already reported in game), any additional evidence, video evidence in this case, must be sent to support here: https://support.deadbydaylight.com/hc/en-us by submitting a ticket and select "Additional Player Report Information".

Regarding the naming and shaming rules, here, just like many other discussion platforms, we do not allow it, since it can lead to witch hunting and on top of that people could fabricate fake evidence just to throw shades at someone.

On the EULA there's a section about exploiting bugs, since this is being done deliberately and not by accident, can lead to a ban. We do not have further information about this.

Disregard my previous comments, after discussing it with my colleagues, they pointed out they have been kept hostage for a long amount of time, which is bannable, so please in a situation like this just record the match and report it properly, following the correct procedure.

That's a different situation, if they do it from the start, you can definitely report them, the situation OP described is different, the killer wasn't blocking them in a corner from the start, the only reason why they have been there for a while was due to the team not triggering EGC.

When situations like this happen, usually the best thing to do is recording the fact and reporting it, however in this specific situation the long wait was mostly due to the teammates not opening the gates and not starting EGC.
Don't get me wrong, blocking you in a building is definitely not a nice behavior and if done for the entirety of the match, it can definitely lead to a punishment, but in the situation you described, they kept you there for a long time because no one opened the gates and didn't trigger EGC, so in conclusion this wasn't holding the game hostage.

I'll get outright toxic messages over the psn with words I cant repeat here. I'm not sure how they would enforce that one. I get they usual survivors teabagging, which doesn't usually bother me that much but it's still there. I get survivors throwing games in various way like not doing any gens at all or sandbagging me or friends I'm playing with in ways that are obvious not to do. I'm talking the scenarios where their bad decision making is obviously more than them being bad at the game. Like when they slug you or your teammate at endgame until one of you bleeds out because they just need it that bad. Like seriously just hook me and end the match; I'm not like this on killer It's just so incredibly unsportsmanlike like. It's like people dont realize everyone's just trying to have a good time and they are ruining it or they like making others have a bad time. Which is toxic by definition.
For things happening in the PSN messages, you have to report them directly to Sony, it's their responsibility.

The reports do something if what you reported breaks the rules, reporting every game seems a bit excessive and I suggest you to read the game rules and see what can be reported and what not, because what you are doing with that is flooding the report system and basically abusing it.

Crossplatform ban evasion
Players who have violated our rules multiple times leading to permanent bans could get their access revoked on any accounts they make if the player is seen committing bannable offenses after the very first permanent ban. The structure and severity of these bans are followed throughout any platform.
Abuse of Support Services
This includes making false support tickets and lying to support agents. Our agents are always here to help the community. However, those making false tickets just to get a step ahead of everyone else and take advantage hinders our ability to better help everyone. Penalties include suspension of your support services, up to a permanent ban of your in-game account.
