Sole Survivor: 15% and 30% are extremely big, maybe reducing to 6/7/8% for solo action and 25/22/19% to altruistic for example. Gen speed is already a big problem but overall it's a great rework idea.
Overwhelming Presence: Remove the extra terror radius, I think that only Distressing should make you have a bigger one (but currently its only 8 1/2 meters for a normal killer, maybe buff it in a future Fixing Worthless Perks?).
Keep fixing, I love to see your rework ideas.

My qualm is that "Mind Broken" would be a new status that is just a weaker version of Exhaustion and otherwise brings nothing new or unique to the table. Reintroducing the old version of Exhaustion in this way would also be likely to confuse newer players, who would wonder why one status lets them recover while running while another very similar one does not. Plus, if the duration isn't intended to be enough to be useful (such as with it's current, hilariously minuscule Exhaustion penalty) then why would anyone want to use it?
If you don't want Mindbreaker to cause an overly oppressive Exhaustion effect, then lowering the maximum amount of Exhaustion it can build up to would accomplish that. There are plenty of loops where survivors can gradually recover from Exhaustion, especially if there's only 5-15 seconds left on the timer. Plus, by using normal Exhaustion you can force a choice upon the survivors: do the gen near the killer and risk being in chase while exhausted (and possibly Oblivious), or move along to find a different gen where the negative effects are less likely to be an issue.
I like the ideas behind how you would have Mindbreaker apply its effects, but I'm not a fan of reviving Old-Time Exhaustion through a killer perk.
The "Mind Broken" status effect is an interesting idea, maybe doing more things than just being old exhaustion would make it better, new players have to learn a lot of confusing effects and i think that a new one would not be bad.

No Mither didn't have Broken as a status effect up to patch 1.6.0.
Nemesis wasn't the first instance of Oblivious, it was introduced with Demogorgon.
I dunno, it just seems weird to have a specific status effect for "exhaustion but you can recover it while running"

Mindbreaker is alright. Probably wouldn't see too much use with that new buff (but a new status effect solely for that one perk seems weird imi)
They frequently add new affects for only a single perk then add to others stuff (add-ons, powers) such as broken with No Mither, oblivious with Nemesis, exhaustion with Sprint Burst. I think its fine

Slippery Meat: This perk would be unbalanced, 50% faster is to fast, combined with other perks such as Breakout or even Boil Over. I liked the idea of converting the recover progression and would make a great combo with Flip Flop. Maybe just lower the numbers.
Mindbreaker: Awesome idea, I had already thought on putting oblivious into it, but the Mind Broken it's fantastic. would be an amazing perk.

I think they need to fix other things first, like:
- Something with the start of the match (like EGC)
- Rework keys/moris
- Rework/buff the Clown
- Delete the Iridescent Head add-on
This game has a lot of problems and matchmaking is not as important as the mentioned above, but it still needs to be fixed.

"You can also slug" says the survivor who uses the small pp combo.
DS is unbalanced, it is the strongest perk right now and makes the survivor INVULNERABLE when used with Unbreakable and Deliverance, it's designed as a anti-tunneling perk, but right now it isn't. if they made some other changes like:
- Deactivates if the surv progresses the game (Gen, Totem, Heal)
- Deactivates if someone else is downed
- Least for 30 seconds
- The timer don't go down while in a chase
I am NOT a killer main, I play both a lot, and as a survivor I use DS almost every match for the anti-tunneling and realize how broken it can be if used in a aggressive way.

Ace forever, It was the first one that I leveled up and now its the only one that I have P3 all perks.
I don't have a main killer, I love them all (except Hillbilly).

It would be interesting, but would have some issues
- It would completely kill the perk Ace in the Hole
- Could be unbalanced, imagine Myers getting a Tombstone Piece out of it
- Also imagine Bubba/Hillbilly getting a Speed Limiter out

Depends, they shouldn't be able to run 5 meta perks, but like 4 meta + a "bad" perk (like Wake Up) I think is kinda nice

Outlast and Alien Isolation are terrifying, Until Dawn and Silent Hill are awesome, Friday the 13th is hilarious.

I think Jeff isn't played a lot because his teachables are mediocre, they don't are good enough to be played as a strong character (Laurie) and they aren't bad enough to be played as a meme character (Quentin), he is just meh.

1st - Toxic survivors who get downed by make your choice and still accuse of camping
2nd - Camping can be a strategy in a end game situation with no survivors killed

I love playing with forgotten perks that synergy together, but can't play without DS (it's OP but with random teamates I get farmed a lot)

With corrupt in game this sound kinda useless, but with a few tweaks it can be a great perk.

Wraith - Bad killer, 2 of the worst perks in the game, bye shadowborn nurses.

Wraith needs a complete rework, he is slow, don't apply pressure and its completely reliant on perks and add-ons.
Piggy's Tempered Timer needs to be deleted.

It's not a small buff it's a MASSIVE BUFF,
but he still is the worst killer in the game because he only gets a pallet down where the huntress/deathslinger would get a hit.

A Nurse's Calling is already a very good perk and can destroy survivors with killers like the Executioner or the Huntress on small/medium/corny maps
However Deer Stalker needs a buff and that is a awesome idea

Love and play all killer (except Billy) right now I'm having trouble with the Executioner adept.
As survivor I am a Ace main, have P3 lvl 50 with all perks on him but play others for challenges.

Midwich Elementary School (Silent Hill)
I like playing as killer and survivor in all of them, but the Silent Hill map its horible for both sides (it has the middle area but 90% is indoor)
but its a [BAD WORD] beautiful realm

The Executioner can leave the trails right next to the cage causing both the rescuer and the rescued gets tormented after the save
so he should not be able to see the cages but should get a notification when the survivor gets saved

I am trying to get his adept achievement but only got Midwich and Rotten Fields (one time with a survivor offering) out of 10 games, he seems pretty good but its been dificult with only his three teachables, some green add-ons and a pretty bad designed map.
Also he has horrible purple add-ons, all his other add-ons are great and have the correct rarity, the Ultra-rares changes his gameplay drastically and are awesome (as intended), but the purple ones are garbage, all of the communs are much better.

Plague its a very good killer, got 2 of the strongest perks and Dark Devotion that is great for a lot of builds. Also Jane got some thicc perks
I suggest you buy plague with shards and spent money on the Nightmare on Elm Street chapter, still, Demise of Faithful is the best one.

Its really only a problem because of EGC. Killer closes hatch and your gen is 99% done... oh well better luck next time.
But the Huntress hatchet one was worse. Down like 100 survivors from 20+ meters or something? They changed it to 20 survivors iirc. I got it when it was 100. The protect 250 survivors or whatever is worse too.
On console still is 100 hatches and its just boring, having to play Huntress over and over again. Same thing with the Bill ones, you have to be so lucky to do the Left For Dead achievement now that Left Behind was reworked.
