Someone was being homophobic in a match around a month ago, nothings been told to me that action was taken. I have taken the time to take videos of people doing stuff provided it to bhvr, still nothing.
A couple of clarifications here:
- Video evidence is not required for something that is being said in the chat, the in-game reporting is the only required step
- Only permanent bans appear on player's Steam profiles for example, so if action is taken and it's a temporary ban, there will be no evidence of it on their profile.

As OnryosTapeRentals mentioned, you can report to Support if you believe they are purposely griefing and targeting.
I am not sure about Twitch ToS, but you can also try reporting their channel to Twitch.

I know that I'm more or less confused on the penalty tier, since I was at 48 hours I don't wanna play till the penalty tier decreases down because if I crash again it's 72 hours
I suggest to wait a couple of days then, in the meantime maybe try to figure out what causes the crash.

The penalty decreases with time, regardless of you being in the game or not.

Teaming is indeed reportable.
If you want to do so, you need to report in game and then open a ticket here giving all the information and video evidence required.
If you are not able to attach the clip to the ticket, upload it to YouTube as unlisted and post the link in the ticket's description.

I'm afraid, but there is currently no cross progression between PC and PS.

The reports do work, if done correctly.
To report someone for cheating you need video evidence, so you can use a software to record your match or something like Nvidia's replay system that allows you to record a portion of your game by simply clicking a button.
You can then report in game and open a ticket here, giving all the required information and attaching the clip to it; if you are not able to attach the clip, upload it to YouTube as unlisted and post the link in the ticket's description.
I'm not saying that the procedure could not be improved, I think most people agree that a simpler procedure could make reporting much more appealing and straight forward, but sadly this is how it is at the moment.

Honestly, i was not going for anything in particular and just played for the first 80hours. Then Malenia became a brick wall and i changed my strategy. Pushed all those levels into vigor and dex. Then i beat her with a +10 Halo-sithe. Took a good amount of time, but was extremely satisfying. Then i got her sword and since then im using that. Learning movesets and punishing at the right time feels amazing. Favourite bosses are currently Malenia (Phase1 NOT 2), Maliketh (whole fight), Draconic Three sentinel and definitely Godfrey.
No magic (because i just didnt understand how to use it lol)
Otherwise, my loadout is this:
R1: Hand of Malenia
R2: Halo - sithe
R3: Eleonora´s poleblade
L: completely empty
Talismans: rotten winged insignia, shard of Alexander, Dragoncrest greatshield, Carian filigreed crest
Quick equipment: Flasks and mixture
Mixture: opaline bubble tear + cerulean hidden tear (should propably change the second tear, it rarely comes into use)
Im now sitting at the final boss. Seems fun so far. Game has been a blast.

As others already mentioned, purposely joining a match to stay AFK is against the rules as it's refusing to participate to the normal gameplay and you can report them for that.
Just remember to record your match, report in game and open a ticket here using the chat bot at the bottom of the page; if you are not able to attach the clip to the ticket, upload it on YouTube as unlisted and post the link in the ticket's description.
