Here are the answers to some of your questions
Why does X character keep going?
For survivors: being murdered is not fun. human survival instincts is what keeps them going, or some possibility of escape. also, their memories are sort of soft-wiped whenever they die. so they only have vague flashes of previous trials. when this memory wipe fails, most survivors instantly go insane.
For Killers: the Entity is not a very fun boss. if you've been chosen as a Killer and you don't kill, you get tortured until you do. most killers are fine with mass murder or have been controlled or manipulated in other ways (Hags unending hunger, Onis rage, etc)
Why don't survivors fight back?
We actually can see this happen in the Tomes. Saku, aka The Redcrane is a survivor (at least, they are now) who entered a Bleed and actually fought the Oni. She cut his arm clean off, but it just regenerated instantly. If a killer isn't already indestructable or supernatural, the Entity just gives them regeneration and immortality. Even if someone did sneak in a gun (which the Entity doesn't seem to allow), Frank could unironically tank a shotgun blast to the face and keep stabbing. At most, it would be like a pallet stun. However, in the Lost Realms (aka everywhere inside the Fog that isn't a Trial or some secret lab) the Entity doesn't seem to care, as various Killer-esque indivuals have been shot, stabbed, and cleaved to death, but maybe they were just brought back when we werent paying attention
How do people survivor in the fog long-term?
If you're a Killer or Survivor, your need for food is basically gone (unless you're the Hag, in which case you're starving forever). The Entity either removes their need for food and water entirely or just keeps them at the brink of starvation to torture them even more. However, Lost Realm dwellers are not as lucky. Vittirio was able to find a food supply inside the Fog to keep him going, and the Observer would just summon Wiskey and other stuff. As for The House of Arkham gang or the Parents, im not too sure. In Sakus story one group of people were described as Cannibals so some Lost Realm travelers probably also resort to this.
How does the Entity know who to take?
honestly just random people for the most part. it takes so many people that if some people instantly give up it doesn't really care. some Killers however, are chosen by the Black Vale/Entity and manipulated throughout their entire lives until they finally become a Killer. it just depends.

Not exactly. Dwight has been in the Trials longer than Vittorio, thats for sure. But Vittorio seems to have been wandering Lost Realms/The Fog for a while. Of course, The Entity doesn't really seem to care about all that human stuff like linear time so maybe it took Dwight first and then timetraveled back to grab Vittorio.
and then of course there is the Infinite Survivors thing....

We last saw him in a video, climbing up set of stairs before walking into a "party" filled with fog monsters. Then he screamed a bunch and vanished. Now we're getting our lore from the "House of Arkham" which is basically a group of investigators living in the observers tower. Its possible that the observer is stuck in some kind of loop by the entity of dying over and over. if you wanna know specifics just read the wiki or get the lore ingame.

they used to do this sometimes, putting in individual random ass peices like... a jeff shirt, meg pants, plague head, trapper weapon, nurse weapon, etc. but i guess its easier to pick an outfit and make it all the same colors (like rift 14 having valentines AND st patricks) than to take 3 random things and make them all different.

Escaping the Entity is possible. Just not through the Trials. Several lore characters have escaped from the Entity.

Except that's literally what I stated in the OP.
Only way you catch up to someone on macmillan is if they A. make a mistake themselves or B. Fall for some janky mindgames, which good players... do not.
So you understand that an actually good killer can mindgame survivors on MacMillian and cut down on loop times... but would rather just stomp through pallets and then win chases that are just running away. "janky mindgames" lol just get better at mindgaming and you will realize how unplayable gideon is

Macmillan is genuinely worse for M1 killers as you have unstoppable jungle gyms within 15 feet of each other and windows you cannot break / stop. You can break the pallets on gideon and get some hits in. You will be stuck in one chase for at least 2 minutes on macmillan maps.
you wont get looped for 2 minutes on millian unless you are really bad at killer because of something called MINDGAMES. you literally cant do those on gideon.
On MacMillian, you MIGHT get looped for 2 minutes if all your mindgames fail.
On Gideon, you WILL get looped for 2 minutes with no chance of making it shorter because of the 30 god pallets.

I wouldnt be suprised if he showed up as a Killer soon. If they set up 10 tomes worth of info about him just to make him go poof then uhhhhhh... that would suck

I think Dredge is actually pretty smart, in more ways than just being smart at killing survivors. The Dredge is probably the smartest at actually causing emotions for the Entity to feed on. If it wanted to just kill stuff, it would run around as an invisible shadow blob, just slurping up any survivor it runs into. Instead, it uses not just an actual weapon, but an actual head and body, just to look scarier. With the doll outfit, it uses dolls because it knows they're creepy as hell.
Plus, just look at how people are reacting to the Dredge in real life. It jumpscares people and makes them scream IRL. It knows what its power is and how to use it to not just to kill people, but to scare them. In the end, the Entity doesnt want a perfect killing machine that slaughters every survivor instantly, because then the survivors would lose hope and get tossed away.
TLDR: It might not be smart in terms of Human Things, but it certainly knows how to please the Entity

as a "Professional" killer who enjoys playing as The Cannibal, i hope more maps change their hook placements to me more like this. It reminds me of the hooks in my home of The Basement.

im pretty sure that around 6 years of Prove in its current state existing in the game would be enough data to show that it lets survivors blitz through generators much faster than normal. Already seeing it a ######### ton since the event, and once gen regression is nerfed survivors are gonna realize how broken it is. Plus, since DH will be obliterated and they wont be able to actually loop or chase anymore, they can use this to substitute their lack of skill.

Sometimes the simplest explanation is the best one. BHVR says 2 Kills 2 Escapes is what they want. BHVR balances strictly around low MMR. Makes sense maps would also follow this strategy.

The shortest distance between two points is a straight line and the DEV tend to take the path of least resistance when it comes to coding. I expect they aren't going to spend the time coding to allow all of us to jump into extremely high BP by selling them back. They will simply invalidate them all and if we are lucky they will give us one of their BP payouts which will be the same for everyone. So whether you have five Mori or six hundred (which I do) we will end up getting an amount somewhere between zero and a million BP.
i just realized this post was a really long roundabout way to see "BHVR should have given me BP when they nerfed Keys"

the darkness killer is hard to see. i think in nightfall survivors should see his aura at all times to make up for not being able to see around the map

Honestly? I think we need to nerf wraith's stealth. My teammates cannot handle it and just hide. But the Wraith finds them anyways and then chases them around. His stealth just destroys soloQ where I can't tell my teammates where he is. I think making "The Beast" - Soot basekit would be good. Hopefully they removed the Dredge undetectable AND the reduced vision during nightfall, so he can stop winning in soloQ so much.

the prestige update is coming this summer, probably during the midchapter. With the anniversary and rank reset coming up, you should have time. during double BP i was able to P3 4 killers (Demo Phead Artist Slinger). It isnt really that hard.
and P3 survivors: Feng. And P1 Jeff.

I've seen a lot of players who talk about the counterplay to deadhard being "just wait it out" but not a single one who has said anything about what to do when DH is being used in a loop or for distance.

I always thought that the Artist's torture was more mental than phsyical. You can't see scars or hooks on her body, but ink drips from her mouth and eyes. If you wanted to make it sound really brutal, you could claim the Entity rotted her body from inside until she was nothing but a shell of ink.

So, the Dredge lore directly states that The Fold was founded in the 1960s. It couldn't have taken more than a few years for Otto to drive the members to insanity and summon the Dredge, and then he has to go find The Doctor, mentor him, and then get killed. So, pre-1965-ish, though the Dredge as an idea could have existed beforehand.
From the DBD wiki: "The Fold was founded on a private American island in the 1960s by a group of anonymous philanthropists."

my definition of linked is usually in terms of power and lore. Ghostface is just a sneaky boi, and the Entity lets him turn off his connection for a bit (though, with Wraith and other stealth killers, I imagine the Entity is aware of this severance, unlike Beast of Prey, Sadako, or T1 Myers.) And with the straps, I don't think floating clothing does that much to get him close to the Entity, in the same way Trappers metal hooks don't do much to seperate him from it.

you say that... like Ghost Face didnt get his name from his lore. like Legion didn't get their name from lore. like Deathslinger didn't get his name from his lore. like Demogorgon (technically) didn't get his name from his lore.
imo, The Tricksters fits well with his design (because it was literally his name) and his power (throwing knives like that is a trick, you cannot deny that).

Emphasis on the Ash part, I've heard of crazy women who burned down their houses with family inside being called Ashen Ladies a couple times but that's about as obscure as it gets and could be a regional thing.
On the gloves in her outfit when she is blinking instead of the usual cracking effect it actually looks like her hand is burning from the inside.
Finally the dress and weapon have an ashy particle effect that makes it looks like she is burning without fire/smouldering.
Also yes her lore is ######### awesome
thats a pretty interesting idea, never heard about "Ashen Ladies" before but i guess you could say the death of her husband burned her life to ash or something.
also, i never noticed the new effect on her hand during a blink, normally she has scratch mark looking things which makes me think that her power is completely given by the entity, and that she basically just throws herself thru the spirit world to hit people harder.
also, particle effects are cool as hell, first wraith and now nurse if they keep this up rift skins are gonna get better and better.
