A flowchart could help
At round start the game checks if the killer uses a hex perk
Where is a dull totem
-> will a survivor spawn next to it -> yes, use different location
-> is the totem in the proximity of a gen? -> yes, use a different locationand so on
also hex totem should only spawn until the first action is done
for example
ruin: spawns on the first skill check of. gen

Dying Light comes into mind as well... You as obsession dies and the rest gets a major penalty.

So if there is a cooldown, can survivors heal the downed person and the killer just has to eat it?

Also certain perks, like Dying Light, actually encourage tunnelling that poor obsession.

When those pesky flashlightes get them smacked out of the hands and the players is confused what just happened. Then tries to get the flashlight back right away and BOOM smack no. 2 and downed -> hook
or when a survivor, who looped you, gets to the gate and then eats tier 3 myers just inches away from freedom -> hook -> end of story

Oh c'mon now... People DC when I smack their precious purple flashlight with red add-ons is just pure joy
And no, it's not wasting a slot. It's very very useful against SWF and I know that very few Killers actually run it. It confuses them so much and sometimes they return seconds later and get a 2nd hit delivered right away... of to the hook with you LOL

For the Killer it's pretty simple: He sees/gets notified the unhook and sees the unlucky injured first. Common sense which one the killers goes after.
I know it sucks to be the poor smock the killer sees first, but that's how it is. Happens to me as wellAnd if you be the obsession and the killer runs for example dying light, well... you screwed

Freddy is fun on paper. But in reality he sucks. Getting a 4K in higher or even high ranks is really tough. Survivor blast through and you just waiting for them to fall asleep. I had like 10 Freddy games today and it was a chore to get them done. It's freaking hard, especially against SWF
Don't pick Freddy if you want to win. I switched back to Meyers and BOOM much more fun and success. -
If a hook is close by: I dribble
If no hook is close by: Back to an obstacle and eat the DS -
You could also just change the color of the totem's aura. make it green or whatever for Haunted Grounds.
As for the other totems... It surely would be nice to know which is which
