I used to think it correlated to survivor lineup in the HUD, but as far as I can tell, it's random.
The fifth offering in the loadout is always the killer. Other than that, it's a free for all.
If the Killer brought an offering, theirs will always be the furthest on the right of the line up. That's about it, though.
If someone dies on their first hook because their teammates don't feel like saving them or is tunneled out before a single gen is done, they're the one who burned a double-Bloodpoints-for-all offering.
(Mostly joking, though it does seem to be a universal truth.)
