Does Plaything have a core totem that can be cleansed?
Or is it more like a non-hex perk that creates a hex totem for every first hook?

Not a core one no. It turns a dull into a hex when a Survivor is hooked for the first time. If their are no dull totems then it doesn't do anything. Only the survivor it is tied to can cleanse it for a certain amount of time. After that time has elapsed any survivor can cleanse it.
No it creates up to 4 separate instances
It creates a hex totem per survivor on the first time they are hooked, and if the survivor dies or dcs the associated totem becomes a dull totem again
Survivors have to break all dulls to prevent the perk from activating. Each dull broken further prevents it from activating.
Seems like an insane perk if it has no core totem.
It kind of has the trade off that it doesn't play well with other hex totems.
except for retribution
Yeah but that's a fine tradeoff. Hexes are kind of throwing the game to RNG as it is, I always hate running hexes.
