It's a different kind of toxicity, though another game might have toxicity in the form of verbal abuse and insults, DBD has less of that and more of in-game toxicity, and while someone pressing control a bunch of times sounds like something that should get you in a bad mood, knowing what they are saying by doing so does get you in a bad mood, it's not exactly just someone crouching a bunch, there's an actual meaning behind it that is pretty toxic and meant to make you mad.
I disagree that dbd has THE most toxic community, but I think it's still a pretty toxic one in general.

I really dislike going against Myers and Doctor, Doctor more-so. Both of them are essentially rinse and repeat; with myers it's just stalk until tier 2, then 99 tier three until you see someone and get a free hook, not really difficult to go against but its just repetitive. With doctor it's just zapping vaults and going for the hit, almost no mind game on the killer's side, and like myers, it's just rinse and repeat.

The only part i'm going to comment about here is the last part referring to letting the killer open exit gate whenever they want; i'm going to assume you mean when 2 survivors remain but gens arent complete (as letting the killer do it whenever would just lead to the game starting with an exit game open and remove the whole point of the objective style game, also would lead to the abuse of end game builds where the killer just downs all the survivors and then blood warden kicks in), so if you are talking about when 2 survivors remain, here's my issue with that: 1, that gives way too much power to the killer. Even as a killer main I can admit that that ability would let the killer control the game a ton, especially with end game builds and/or characters like trapper or hag. 2, it removes the opportunity for more objective category bloodpoints for the survivors, which seems a little unfair.

While I understand your reasoning, if all hex perks were buffed to be amazing, people would just run a buffed hex perk coupled with two hex protection perks, ensuring the OP perk never becomes broken or if it does it costs survivors a good amount of time/hooks. For hexes to be buffed a lot, hex protection perks would need to be nerfed heavily or reworked/removed. It makes more sense to just make hexes harder to find or take longer to break, as this way they avoid having to rework a ton of perks.

That's what I was thinking, but regardless of that, I doubt BHVR would even be able to ban someone for this if they wanted to, as I know it's possible to do just on some monitor's hardware settings and/or monitor software.
(btw even if this in specific is only possible using whatever software that guy was using, it can be closely replicated with the utilities I mentioned)
