For the first one, i think you're just aiming wrong. For the ring, it's kinda odd, but say your in a chase, sometimes the row of flames won't be directly in front of you, they will spawn to the slight left or the slight right and in the general direction of the survivors, sometimes. I recommend just not using it, I think the travelers hat and clock tower gear are better.
