Why do many survivors suicide just as you are about to unhook them?

1) Maybe there was a lag. If there's a lag, you die. That's just how it is programmed.
2) If you are the last survivor, maybe they want to give you the hatch and were just struggling before to get some BP.
3) The killer is nearby and they don't want to get farmed.
Most of the time, they're annoyed at something you did earlier. Which might include farming them, crouching across the map or hiding in lockers when the killer is nowhere nearby, letting them go straight to second struggle state, etc.
Or just blaming you because they suck.
