The "struggle effect" refers to the way the Killer sways as you wiggle. This perk is not supposed to speed up your wiggling, and it's not supposed to guarantee a free escape every single time.
There is only 1 perk that affects how fast a survivor can wiggle free, and that's Iron Grasp (which makes it take longer).
Boil Over is alright. It'll make risky hooks much harder to get but close hooks are obviously not gonna be stopped.
