Pyramid Head changes.
It's up in the air so here we go. PH has a slew of problems from immediately murking people stuck in animations to not getting anyone afflicted with Torment so here's a few simple changes.
When performing Punishment of the Damned, it will only spread the trenches created by Rites of Judgement. If the survivor is already Tormented, then PotD takes a health state away.
Not only will survivors become Tormented more often, but the damage from the attack will now have to be built upon first. And adds yet another layer to the question of Cage them now, or save it for later. It will also become an active way to actually curse survivors.

Survivors are normally tormented until they get the cage. This'll just require you to get them tormented first before Punishment damages them. But it will continue spreading trenches.
It's like a mix of Plague and Doctor.
Less impactful? It's an instant hook state disregarding every perk in the game when you cage people. And every add-on that deals with it gives you boons like seeing auras or hiding your prescence.
It's underutilized because of Punishment not needing any conditions to work. Besides, the trenches also are very limited in coverage. They can only cover where you have physically gone. If Punishment created trenches it would be a lot more flexible.
It's extremely useful, PH can tunnel a survivor freed off the hook, and then cage them circumventing DS, and then upon finding the freed caged survivor, they can then again completely ignore another hook and just flat out mori them. His basekit is broken.
Unhooks don't matter in the general scheme of things. Hooking isn't supposed to be a one n done. They lose a hook state and you get back to patrolling gens and applying pressure quicker. And that's also one survivor who has to drop everything to save them.
