Has anyone else had the Entity decide to eat your item when you escape?

My running tally is: 1 emergency medkit, 1 ranger medkit, 1 mechanics toolbox, and 2 engineer toolbox. whether they are brought in or found, you randomly lose them despite escaping and the result screen clearly showing it in your hand. It doesn't matter if you use the gate or the hatch, and even a white ward won't protect you. I'm on PS4, anyone else run into this?

    It happens occasionally, mostly to my purple flashlights for some reason. Not sure why, but it’s whatever, it’s just an item, not like it somehow bugged out and stole my perks. Now THAT would be scary.

    Yes and most of the times I had really good items
