I just think I figured out what causes this bug to occur, as myself and a friend have been running Blast Mine the past few times we've played. So the way I think it happens is, the killer kicks a generator affected by Blast Mine(the killer get's blinded), the killer then chases a survivor and downs said survivor(using Jolt), the Blast Mine(for some reason) stays on the generator(even though the generator isn't highlighted anymore) and goes off again when the generator explodes from Jolt. Haven't seen if it works with Eruption or Scourge Hook Pain Resonance as of yet. If I do I can update again. I hope this helps.

Just watched the video a couple times in slow motion, the Blast Mine flashbang is still spawning on the generator as the killer downs the survivor, that is the only thing I can think of that could be the cause of the bug. It could just be the fact I placed the blast mine milliseconds before the killer downed the survivor. So the Blast mine exploded with the gen. I hope this helps.
Also the fact that the screen was pure white is weird, that's the way the screen was, when you were blinded from way back.

Update: I did further testing, I tried to replicate this scenario, and had no luck so far. I've tried spawning the basement in main building, with the map set to Grim Pantry, and the hatch to spawn at Killer Shack as it did when this bug occurred. I just can't seem to replicate it. It may just be that one tile for the swamp map with the hook by the pog log, if it spawns next to killer shack and there are no exit gates around it, and the killer shack is set so the window is facing main building. That's my only guess as to why the bug happened. I hope that the team can investigate further and hopefully find what caused it to happen so it doesn't happen again. Best of luck. I will continue to test on my end to try to replicate it, if I do I'll update again.

Ok so I restarted my pc and verified the game files and now I can play again. Don't know what caused this, but I hope it doesn't happen again.

Update: Almost an hour later and still can't get into the game. I get to the "press space to continue" screen. I press space or click and the game closes.

I just had a match disconnect mid match, I was playing killer and "lost connection to host."
