My game gets blurry (PS4)

My game gets blurry after I take a hit from a killer. Anyone else experience it? I've heard PS4 has poor optimization, but even then, I think it might be with the TV. The only counter argument though is that it only happens during events, and nothing else.

Any ideas how I could fix this issue?
