Server disconnects
When you're disconnected from the server, the game recognizes that it isn't an intentional DC. However, any progress made toward challenges, experience, etc. is nullified as if it never happened. If this information is catalogued, we should be able to keep it. As is, server disconnects are common and it ends up wasting a lot of our time.
