Steam - Hook counter sometimes bugged

I've noticed that sometimes in a game, usually custom, the hook counter will be incorrect. Sometimes I have 2 survivors on death hook but it says only 1 hook left. Another time, I had 2 survivors alive, both on death hook, and it said I had 3 hook stages left. I have images of the second circumstance. In the attached images, both Meg and Ada are able to be mori'd but the counter shows only 9 hooks. It may be related to the fact that all the bots have Up The Ante, making them almost always try to attempt on 1st hook and often going into second stage, but that's just a guess.
