Dredge need False Hydra legendary / Ultra rare skin to happen
So, there's going to be a bunch of spoilers for this DnD infamous monster, so if you are playing DnD and experiencing some of the following things (you remember people nobody else does; the town your characters are staying looks a little bit too empty; animals look horribly distressed; people suddenly forget people they aren't supposed to, etc) in your campaign, probably stop now.
I know this monster is basically based on Dead Hand from Zelda, so maybe it makes more sense to want Dead Hand legendary skin to happen, but I think False Hydra has separated from this design and acquired a lot of unique features of its own to be a good candidate instead.

To put it simply, False Hydra is an abomination that feasts on human beings in their homes, slowly consuming everyone it can reach and growing exponentially like a cancerous tumor right under everyone's nose. False Hydra is not mobile and can't properly hunt or fight, however it has a unique ability, allowing it to hide in plain sight.
False Hydra is able to sing a mind song. Once it grows big enough, it starts singing. Whoever hears this song, is ignoring False Hydra - they could be looking right at it, hearing or even touching it and they would still not suspect a thing, as False Hydra's long necks with pale heads sprout from the ground like flesh plants, watching everyone and looking for their next prey. It could be looking right at you and you would look at it, but your consciousness wouldn't acknowledge that.
Moreover, whoever is devoured by false hydra is also forgotten and ignored. People would never remember a person they lived their whole life with, although part of their mind would be screaming something is wrong. They would see the missing person's stuff and come up with the craziest excuses why is it there, just to keep the consistency of their subjective reality in place or they would straight up get rid of things that make them remember. In some False Hydra interpretations, this is a separate ability, in others, it's the same ability that is acquired because whoever is devoured by False Hydra, becomes part of it and sings with it, sharing its ability to erase itself out of other's minds.
False Hydra looks like a big grey, pale skinned blob of flesh with unnaturally long thin necks and flayed flaccid faces on them with wet holes instead of eyes and big mouths with no lips and with human teeth. It also has arms and claws growing on its body in random spots that it uses to slowly crawl from place to place or finish off its prey.

This monster is not official and the designs are not set in stone, but I think those give a pretty good idea how this monster looks like. It has a lot of similarities with Dredge in its visuals, like they both have those weird claws and long thin arms that are growing from the base of the monster. In some ways, both Dredge and False Hydra are amalgamations that consists of conjoined human bodies and minds.
Dredge's nightfall features a literal choir of suffering souls wailing in unison as the nightmarish creature they were turned into continues to slaughter more. When Dredge attacks, you can also hear a lot of voices screaming in unison, which is a huge similarity with most False Hydra's interpretations.
Dredge's False Hydra skin would probably mainly change its body carcass part. I imagine it could change the head into 2-3 separate heads with necks +- as long as Dredge's neck and moving independently, while a few other heads a taking the place of the skulls on its back.
I don't think its weapon or second arm would change that much, except being more greyish and maybe thinner.
