Possible bug with Rites of Judgment trails.
I'm not sure if this is intentional or not. I first started noticing it during the PTB where Pyramid Head got his zoning changes.
If a Survivor walks onto a Rites of Judgment trail, the piece they walked on will disappear. Other pieces of the trail will stay as the Survivor walks over them until they leave the trail and stop triggering Killer Instinct, and if they walk back into it, that specific piece will again disappear.
I don't have any video evidence at the moment, but I will see about collecting some.
