First game ph and already experienced 2 bugs

I Just played my first game of ph. The first bug i experienced is that when a survivor helper someone out of the cage there is no sound

Also 1 survivor i couldnt Mori even when she was on dead hook and was tormented. She Just got sended to the cage and dies immidiately

Does everyone else experienced this too?
