Something that confuses me about the exhaustion mechanic

Because if they do that then Survivors will somehow no longer die on 3rd hook and 2 killers will spawn in the same game.

Have you heard Feng after she runs normally? Woman sounds like she needs to be attatched to an iron lung after 3 seconds!
