Self care is a selfish perk you can run better perks to help your team in many other ways.
Many times healing right after being unhooked is the best way but if you don't feel safe just point it out to a safe location.
I always play swf and none of us use self care. This is a perk for beginners until they get used to take a hit and escape from the killer.
Plus if you have a solo he doesn't know your agenda and perks you're running. It can be frustrating but I'm sure he was also frustrated for trying to help you after he was the one who unhooked you. Maybe he was a baby survivor.
After unhook someone if they don't want to be healed right away I just let they do whatever they want and most of the time they get caught injured while trying to self heal themselves which takes longer.
Just food for thought.

Playing last night as killer it was so stressful. It was clearly that that player was speed hacking. That is the issue I encounter very often Playing as killer. They always out run the killer like it is impossible to get closer to them. That is one of the reasons why I enjoy playing survivor more often. Although I can't out run the killer but I can dodge and make them lose me.
