• I have actually encountered the same bug. I didn't have any addons to mess with the escape, the build didn't have anything to mess with the escape as well. The last survivor was injured, and during the hatch escape animation I threw the ax at her. It did hit. The survivor's model remained under the hatch, making injured noises. So it could be that losing a health state during hatch escape animation from The Animatronic's fire axe is the cause for the escape to not count.
