As a VPN user almost all of the time (as evidenced by my username) I agree that a "skilled" VPN user can use it as a sort of lag switch. They don't even have to connect to geographically distant servers, they can simply connect to a congested server and they can throttle their pings to victory. It takes considerable skill to learn how, though, and personally I find it dumb that people would want to learn how to compute in their heads to know if a hit should land or not based on their ping, especially (as HollowGriefs had said) since the lag isn't a one-way street. They could have used that time to just learn to play the game well like normal people do.

I support GoodBoyKaru's answer. There are only a few exceptions, such as when the VPN-user is on a game-dedicated VPN server (as some companies offer), when they're using a VPN server that is geographically close to them, or when he is on an exceptionally fast connection.
Then of course, it could just be bad connection on the killer's end, as what appears to be the case with your last example.

The thing with VPNs is that a few of them offer static IP servers or dedicated IP add-on services that will make them pretty much undetectable to games such as this. Typically, services catch VPN users by watching their IP addresses -- VPNs assign IPs randomly, and a user with a frequently-switching IP is flagged as a VPN user. Some services also know exactly which IPs come from VPN servers. But when a VPN-user upgrades to a static IP (which some VPN providers offer for free), then services can't catch that. That said, even if the game starts banning VPNs, it's just a step up for VPN users to make their activities undetectable. That means, of course, that you can use the same for your own ends, but I'm not sure how healthy that will be for the game.
