As annoying and frustrating as it is to have a DC, the fact of the matter is that people are going to do it no matter what the penalties are. You don't know why people DC. Maybe it's because their internet cut out, maybe something came up irl, maybe they just got so mad at the game that they needed to take a break before it escalated. You don't know their situation, and imo it's ridiculous to permanently ban someone for something as trivial as that. This always has and always will be a casual game, and while it really sucks having a DC, there's nothing much else you can do other than either play it through or if you really want you can always just let killer kill you to move onto next game.
I think a better solution would be having a very small time window where someone who may have unintentionally dc'd (server error, lost internet connection) etc. can join back in the game. Or this might be quite complicated/silly to implement but perhaps bots could be added as a replacement for w/ the same amount of hook states as the survivor who'd previously left. It still wouldn't be ideal as it's a bot and not an actual person but it's better than nothing.

That could be it too now that you mention it, but in either case whether it's the teleporting or the add-ons, it's a bit silly to me that they've acknowledged a bug like that and haven't fixed it yet, considering it's apparently been an issue for months.

Very late to this post, but I believe the add-ons that are bugged are his yellow add-ons. I got merciless my first try at his adept but did not get the achievement, saw a post on reddit saying it was the yellow add-ons and tried my next game without yellow add-ons and got the adept achievement.
First game I had on Burnt Letters and Sacrificial Knife, second game was Broken Doll and Sacrificial Knife. I'm unsure if the bug is with all of his yellow add-ons or just Burnt Letters, but I used Sacrificial Knife in both games so it was definitely switching the yellow to a green that seemingly fixed the bug. I also was streaming for both games if showing the recordings would help resolve the issue.

Just had the same issue with this ritual last night (as well as the achievement "Even Punishment" which has similar requirements). Tried to do it two games in a row, and even made sure I saw the "Phantasm Attack" bp event on the top right after teleporting and hitting but still couldn't get either the ritual or the achievement done. I am also on Steam with Windows 10, so I'm not sure if it's just a PC issue or if it's on console as well. I was streaming at the time if showing the recording of the game would help in resolving the issue.
