I even farmed it with survivors to see if it was possibly bugged and it still didn't pop. It got to the point to where "Phantasm Attack" wouldn't even come up anymore and it just became a normal hit. There's for sure something wrong with this stuff counting.
I’m having the same problem on PlayStation. I even worked with the players twice, and still nothing. It’s one of my last trophies.
