What bad inputs have you run into while playing?

We've all probably been sucked into a locker while trying to vault. That's a common one.

I've also had my character try to open a chest instead of jumping through the hatch. (twice)

Swapping items on the ground when trying to heal someone after unhooking is pretty common.
