why do I do the quest the doctor challenge "Mad about you : Infflict Madness I on 4 different Survivor" tome 2 level 2 cannot be done?

PLATFORM: PC

STEP BY STEP INSTRUCTIONS ON HOW THE ISSUE OCCURED: I've been doing this challenge for 7 games and the quest can't be completed
