Eye of Vecna sometimes doesn't work?

literally my first chest on my first attempt against Vecna I bink a 20 and get the Eye of Vecna. Soon after got into chase, rushed out of a locker at full health, and wasn't invisible or speed boosted at all.

I did attune or whatever to the eye.
