Visual bug regarding hook states

When being hooked for the first time and making it to second stage, after being saved from the hook it does not visually update how many times you've been hooked. It still will only show you being hooked once

This has happened on every character I've played and when playing with friends they also notice this.

This results in confusion since we think we have another hook state but we don't, so we die on hook.

I am on PC have had PS4 friends that also noticed this

Edit: Still occurring after multiple days on every survivor played as, and against every killer. Still seems to be tied with game not updating after reaching 2nd stage on first hook.
