Is the "out isn't an option" challenge bugged?

I did what the challenge said by not letting any gate opened while using no way out, yet the challenge is not completed. Is it because i spared the last survivor by giving them hatch? Do i need to sacrifice all survivors?

Also there's a bug in RPD map where you can't snuff boon totem near the hook in the third floor
