Why didn't I get Adept?

Just did an adept pinhead attempt and after a really good game for me I got merciless killer but no adept.

I checked my emblems and it was 4 iris so it wasn't a fake merciless, I also double checked my perks to make sure I didn't equip a wrong perk by mistake.

did the game bug out or did I do something wrong?

    I have no idea.

    I got the achievement just fine, but this is apparently a bug that a lot of people have been having.

    A lot of people have also been getting it without incident, so I have no idea what the issue is here.
