I know the perk got changed a long time ago but

Is the 75% stagger reducing something permanent? Does it count even when you're exhausted?

Does the grunt removal also work when the perk is in cooldown?

Or do NONE of the effects of the perk work when in cooldown? I know the stagger used to be unlimited?

    Yeah, the unlimited stagger removal created infinites on several maps, so it was nerfed. Now the falling noises are permanently removed with the perk, but the stagger is only while not exhausted.
