Original Pain addon not working

Used this addon on pinhead 3 times now to check and it never inflicts deep wound on a survivor

I have used this when

1 - survivor was out in the open with nothing to help break the chains.

2 - survivors had objects that helped break the chains.

If this is the case, this addon is either bugged or is absolutly useless as if survs are breaking the chains without holding M1, i can never hit them anyway and the chains are more of a hinderance to me than a help.
