Pyramid Head's M2 has a bug
If you change the setting for killer power to anything that is not M2, Pyramid head will instead do a lunge attack instead of Punishment of the Damned.
Here is a video showing how to do the bug: https://www.youtube.com/watch?v=2MiRkfb7tjE
It's Unlisted so you can fix it without people knowing about it.
