Issue with Doctor and Survivors Screaming

Was playing Doctor on new Rotten Fields. Was running Infectious Fright and survivors would scream when that perk activates. But when they screamed from madness tiers, they did not reveal their location with the bubble. Or when the bubble would appear, it would appear in impossible locations, like inside an exit gate when there are 4 gens left. Checked after game and no one ran Calm Spirit or any other perk that hides their locations.
This occured throughout the match and at the end when I had 2 people in the dying state, I could hear them scream but they would not reveal their locations. I was not in line of sight and I could hear them from different parts of the maps.
Been inactive recently so if screaming or madness has changed someone LMK
