I was crawling around and the killer closed the hatch. Adrenaline did not heal me one state, since I was still crawling.

Nope.

I was crawling and my last teamate got killed.

We only had 2 gens done.

for example, in case you remember you ran "for the people" or you know you like running perks that inflict broken. EDIT: or the killer had something that can inflict broken, like the Legion Pin or Pyramid heads Forced Penance perk

the broken status effect ignores the adrenalin heal, if it wasn't a bug this is the only explanation

Broken only prevents being healed to healthy. You can still be healed from dying to injured while broken (No Mither. Other people can still heal you, not just yourself) so Adrenaline should've still worked.
