Trickster knives do not draw blood?

I just received the Hemophobia achievement on my second match against the trickster today. While I did not actually lose a health state, I know I was hit with at least two knives. Does it only count as blood loss if you lose a full health state? Not sure if it counts as a bug, but I feel like I cheated on that achievement🤣🤣
