I have over 400 hours mostly with survivor and, to my shame, have never successfully blinded a killer while it's carrying a survivor. What perks and add ons would help me finally accomplish this? Any tips on the timing or where to aim the light beam would be appreciated as well. Thanks in advance.

    Actually, it only requires your skill with a Flashlight and aiming at head. If you wanna go for Flashlight saves, run Sprint Burst to make it always on time. 
    Remember to flash when the Killer puts the Survivor on his shoulder.

    I don't do flashlight saves myself but from observation while playing the killer. Survivors with flashlights tend to stay near chases so that when the killer finally downs the survivor, they come in for the save. Also don't start pointing the flashlight too early or the killer will be blinded but not drop the survivor they pick up. Try pointing it right as the pickup animation is about to finish.

    Right now some inconsistencies in the character model make it so you should usually aim around the killer's neck to blind them (not too far from the head though). But the current PTB is testing out making it so you just aim right at the face, so hopefully that will roll out sooner than later.
