Keep on getting disconnected when playing with survive with friends

I have been having this problem for almost 6 months or so and its getting pretty annoying, I can get off 2-3 games with my friend before having to close down the game and rejoin. I really don't know what I should be doing at all and I would love some help from this into fingering out what to do. I have my log attached to this post so that someone could help me or something.
