Deathslinger's harpoon not hitting survivors

Basically, this week I had two instances of the DS harpoon not hitting survivors when it should have.
First time was in Ormond, I was on the 2nd floor of the main building and shot a Bill who was on the open, one level below me. The harpoon disappeared in his body and he didn't get hooked. He was injured.
Second time was on the swamp, I shot a survivor who was dropping from a 2nd floor before she could touch the ground (not trying to brag, I promise). She got hooked but the moment she touched the ground the harpoon disappeared.
It's pretty annoying to be honest.
