DS not working against Legion

I hit Decisive Strike against Legion,

got the chime that I was successful, but instead of dropping me Legion carried me to a hook and hooked me. Is there a perk or add-on which overrides a successful DS achievement? Or is it another bug in the game?

Devs? Any response? It is hard enough to hit DS with the lag, but to hit it and not get dropped is pathetic.
