there been times when I went through the hatch and got an adept and for some reason it doesn’t with Adam? Is that a bug or has it been changed that you have to go threw the gate?

    It doesn't.
    Tried doing adept nea some good months back and got out through the hatch and it didn't count.

    Many of the achievements requiring an escape must be done through the Exit Gates to count, unless the Hatch is specifically mentioned in the achievement.
