Blight Serum stops working

It says in the tooltip of it that you will get 1 additional charge each time you hook a survivor, after the 3rd or so hook i did as legion, it would stop giving me charges.

I did not have any charges stored up as all i could do was frenzy.

This was on PC

And it was a 12 hook game.
