i just bought the nurses calling perk in the shrine

You still have to find it in the bloodwebs of your killers.

Ah so even though I bought it I still need to be red perk level to use it

You bought it, so you don't have to level up your Nurse to level 40 to unlock it that way. It will now appear on your killers bloodwebs for you to get.
