Dying in cage glitch

Lunairtic

Hello there,

I was sent to a cage in second phase right after I went to it but then it suddenly killed me and gave me the skull as if Pyramid Head did a final judgement. Please fix this glitch. I have video evidence but have uploaded it yet.
