Possible bug, or something missing from Rites of Judgement or Blood Pact

Against The Executioner (Pyramid Head), when the killer was a great distance away from me and the gen me and someone else was working on, was on the gen for about 20 seconds by the point this happened. The closest trails had already vanished from the area. Then the obsession gets tormented, and then I see a pop up of some of the torment appear randomly around the gen and I got tormented, but the Meg next to me didn't.
I'm not sure if this is: A unlisted side effect of Blood Pact, something unlisted in Rites of Judgement, or a bug that I can't explain.
