Legion Hit Not Registering

Had an instance both in feral frenzy and basic attack that it the hit did not seem to register.

For example, at the end of the match there was a survivor in deep wounds and still at full health. The basic attack non-register happened when stunned by shack pallet.

So far it has only happened to me in one game on two different players. I forgot to look at the items/perks they were carrying but it did not appear to be the insta-heal perk. (No-one was broken.)
