Yes, the "red stain" is not omni-directional, it's affixed to the Killers front view, no matter where they look. You could look up, down, left, or right, the red light will still shine forward towards where-ever the Killers body is facing towards.
For example, when you move left, your right light moves left with your since your head doesn't turn while your body is facing the same direction, instead your whole body turns as a result. Moving your head up and down is different, since it does move on it's own without moving your entire body, the only issue is that your body is still facing forward, which means the red light will always face that way since it's not actually connected to the Killers head movements.
Think of it like this, the red light isn't connected to the head, but rather the front of the body itself.

If it was omni-directional than it would make mind-gaming a lot different yes, but sadly it's not, so moonwalking is the best you can do to fake where you're moving to.
